    Is it allowed that Google Adsense ads may be placed in this slots?

    1) Contact us Page
    2) A page collecting info from user with submit button
    3) Showing info such as "Thank you for sign up" pages including some other content

    Not allowed on number 3, basically it's the same rule as no ads allowed inside pop-up windows. Adsense can only be placed on a page that a user has a choice to visit or not, because the 'thankyou' page is automatic the user is forced to see it. Same applies to seperate login pages, again you are forcing a user to go through that page to visit somewhere else.

    Adverts on page types 1 and 2, I do not think are a problem. However it's highly likely that such ads would perform badly and it's doubtful anybody would click on them. Why would a user who wants to fill out a registration form in order to get into your website or recieve some percieved reward want to click a link taking them away? Same applies to a visitor using the contact form to a lesser degree because they are on that page for a specific reason.
