How to find mysql database

I am setting up php nuke for my website through you guys. I have a problem though. I am installing php nuke and it has a web installer and it ask me for hostname,mysql username and password then mysql database name. I can having trouble where to find the host name aka the local host and I am having trouble finding my username and password for the mysql database. I will make a new user say kraze and with a pass of fluffy. then it says user jj1994_kraze as my username and I am confused if i should use kraze or jj1994_kraze for my username?

use jj1994_kraze for username. and the same format for database name
