Question regarding ad placement

hi, i have a question about ad placement..

do the ads need to be in virtually every page in my site (.htm, .html, .php, .inc, etc..) or just the pages that are visible only from my site?

for example, in verifying my account in google apps, i uploaded a file named googlehostedservice.html with just the verification number inside the file.

supposedly that i didn't delete that page from my site and assuming that nothing in my site links to that html file, will i get suspended for it?

thanks, i just need to clarify what "every page on your site" means.

The pages need to be visible on pages your users will go to. Not having it on googlehostedservice.html is fine since people wont go there.

oh ok.. so does that mean that only the pages that users are likely to go to are the ones that must have ads?
