Form Processor, 100-1000 points

I've been searching and searching for a form processor and I have found 100's but not what I want. What I need is a form processor that will process my Html form and output it as an html e-mail (almost so the e-mail will look like the form itself). All of them that i have found are text e-mails. I need to be able to customize the html e-mail being sent also. Note** I need it to be free.

If you know of a script please let me know and I will reward you with 100 points. If you cant find one but can make one.. I will pay 1000 points.

Well wasn't Corey working on ASP support a while ago...I know a tutorial was posted about installing ASP on Cpanel but never heard whether Corey would try it or not. I think there is also a form builder built into Fantastico.
