File "Submission" Script (1,000 to 2,000 Points!)

Hello, I need a media file submission script. I want it so a user can submit a video or picture(s) and have a title on them and description. It must also be able to read off a file for the allowed "extensions." I will pay 1,000 credits for this. If you want 2,000 credits, I will give you a temporary ftp account and sql account (if needed) and you can install it into my website. Also, If you find a pre-made script I will only give you 500 credits.

No i mean just a simple upload script, so this way someone can upload a video or picture and i can put it on my upcoming website if it meets the required categories. I need it like a file upload script but for picture and videos.

EDIT: I never knew about phpmotion, i was just looking at it and I kinda like it, I will donate 1,000 creds to you cause its a nice find.

EDIT2:nvm, phpmotion wouldnt work correctly for what I want to do, so this is still up for grabs
