Basic Javascript Program needed!

Hello Everyone!
If you know basic Javascript and HTML, you can probably help me.
I want a calculator that will take two form entered fields and calculate the one divided by the other and display that to the user. I am giving 50 credits to anyone that can help me with this.

I will try to use that to guide me to code it myself. I know HTML but don't know JavaScript so I hope it won't be too hard to find out. ;]
