need help with wordpress contact page

I have downloaded a contact form plugin for wordpress cforms II from delicious days to be exact. and it dosen't show up properly. I would like to if possible create a form that has a selection option for where the email is to be directed, like help question sales or add link. Is this possible? or is it not worth my effort?

I don't use contact forms on my WordPress blog currently but I did try Wufoo.com's contact forms in the past. They are a snap to create and implement into a WordPress page. You should check it out if you want an easy-to-use contact form. Sorry I can't be more of a help.
