could some one point me to a free script or tell me how to make a paypal payment form,

features needed in this form is

a domain name field and a drop down so they can choose a sertine area of my website

I.E im wanting this form for sponsor listing so im wanting them to place there domain in a field and the category were they wish to have there site shown on

any advance information would be great
