Permissions Error

Users on my phpBB forum are having permission issues. Files are set correctly as per phpBB requires but they get this. It also happens on the ucp.php file

Has it worked in the past?

I had a similar issue with SMF a few months back where I needed 775 on the 'attachments' folder. 755 wouldn't stick via FTP, or in cPanel file manger.
An admin needed to set mine manually.

It happens on and off. According to the phpBB forums it's an issue with mod_security.
